Emergency and safety notice
This service is intended only for non-emergency health concerns that may be addressed through telehealth.
If you believe you are experiencing a medical emergency, call 911 immediately or go to the nearest emergency room.
Telehealth is not a substitute for in-person care in all situations.
Arbitration and class action waiver notice
These Terms include a dispute resolution and arbitration section that affects your rights.
In most cases, disputes must be resolved by individual arbitration and not as a class action.

These Terms
These Terms and Conditions (Terms) govern your access to and use of this website and any pages where these Terms are posted (Website), plus any related online tools, portals, forms, messaging, scheduling, and payment features made available through the Website (Platform).
The Platform and certain administrative support functions may be provided using technology and operational support services from a third-party telehealth operations provider. If you access the Platform through a co-branded experience, embedded page, or link from another site, these Terms still apply.
By clicking “I Agree,” “Continue,” “Proceed,” “Register,” “Sign Up,” or a similar button, or by otherwise accessing or using the Website or Platform, you agree to these Terms and any policies referenced in these Terms.
If you do not agree, do not use the Website or Platform.
Definitions
For clarity, the following terms have specific meanings in these Terms.
Company, we, us, or our means Patriot IV LLC.
You or your means the individual accessing or using the Website or Platform.
Clinician means an independent, licensed healthcare professional and or clinician group who may provide clinical services to you.
Third Parties means entities other than us, including Clinicians, pharmacies, laboratories, shipping carriers, payment processors, and technology providers.
Content means text, graphics, images, audio, video, documents, downloads, trademarks, logos, designs, software, and other materials made available through the Website or Platform.
User Content means information, text, images, videos, messages, documents, and other materials you submit through the Website or Platform.
Website content and informational use
The Website may include general information about services, telehealth concepts, wellness topics, conditions, or products. This information is provided for general informational purposes only.
We do not provide medical advice through the Website. Do not rely on Website content as a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of a qualified healthcare professional with questions you may have regarding a medical condition.

What we do and what we do not do
We provide technology and administrative services that help users access telehealth-enabled care and coordinate related non-clinical functions. This can include account creation, intake collection, scheduling support, customer support, payment processing, and communications tools.
We are not a medical group and we do not practice medicine. We do not provide diagnoses, treatment, or prescriptions.
Clinical services, if offered through the Platform, are provided by independent Clinicians. Clinicians are solely responsible for clinical decision-making, including whether telehealth is appropriate, what care to provide, and whether to prescribe medication.
Platform support and support contacts
The Platform may be powered, supported, and administered in whole or in part using technology and operational support services from Patriot IV LLC, including for technical support, intake collection, scheduling support, communications tools, and payment processing.
Patriot IV LLC provides general and non-clinical support for users, including help with account access, basic navigation, general questions about programs or services, and customer service issues that are not medical in nature.
Patriot IV LLC provides medical support exclusively. Medical support means facilitating access to clinical services and helping route clinical questions through the Platform to independent licensed clinicians.
Patriot IV LLC is not the patient’s treating clinician. Clinical services, if offered through the Platform, are provided by independent licensed clinicians and or clinician groups (Clinicians). Clinicians are solely responsible for clinical decision-making, including whether telehealth is appropriate, what care to provide, and whether to prescribe medication.
If you have a medical question, you should direct it to your Clinician through the Platform. If you are unsure how to contact your Clinician, contact Patriot IV at support@Patriot IV.io for assistance routing your request.
Separate telehealth consent
To receive telehealth services, you may be required to review and accept a separate telehealth consent. That consent is incorporated into these Terms by reference.
Changes to the Website, Platform, and these Terms
We may update the Website, Platform, Content, and features from time to time, and we may suspend or discontinue any part of the Website or Platform at any time.
We may update these Terms from time to time. Changes are effective when posted on the Website or Platform. Your continued use after changes are posted means you accept the revised Terms.
Availability, eligibility, and verification
Services are not available in all states or jurisdictions. Availability can change due to clinical, operational, or legal requirements.
We may request proof of identity and location, and we may decline service if we cannot reasonably verify eligibility.
Account registration and security
Certain features require an account. You agree to provide accurate and complete information and to keep your login credentials confidential.
You are responsible for all activity under your account, including activity by anyone who uses your credentials.
You must notify us promptly if you suspect unauthorized access.
Electronic communications and e-sign consent
By using the Website or Platform, you consent to receive communications electronically, including by email, phone, SMS or text messages, in-app notifications, or secure messaging.
Communications may include service notices, account updates, reminders, required disclosures, follow-up requests, and instructions.
Message and data rates may apply. Message frequency varies. If SMS is offered, you may opt out by replying STOP, but opting out may limit your ability to receive important communications.
You agree that electronic signatures, acknowledgments, and records have the same legal effect as paper documents and handwritten signatures.
Not for emergencies
The Platform is not designed for emergencies and is not monitored for emergency messages.
If you have severe symptoms or believe you may be experiencing an emergency, call 911 immediately.
Your responsibilities and accurate information
You are responsible for providing truthful, accurate, and complete information, including medical history, medications, allergies, symptoms, and any requested photos, videos, or documentation.
You are also responsible for monitoring your messages, responding to requests, and following Clinician instructions.
You understand that incomplete, inaccurate, or delayed information can lead to delays, denial of service, adverse drug interactions, allergic reactions, or other harm. You agree not to hold us responsible for outcomes caused by inaccurate information you provide or failure to follow Clinician instructions.
Telehealth limitations and clinical appropriateness
Telehealth has benefits and limitations. A Clinician may determine that telehealth is not appropriate for you, may request additional information, may require labs, may recommend follow-up, or may direct you to in-person care.
We do not guarantee that you will be matched with a Clinician, that a Clinician will provide any particular treatment, that a prescription will be issued, that any medication will be prescribed, or that any medication or product will be available.
Prescriptions, pharmacies, and fulfillment
If a Clinician determines a prescription is appropriate, the prescription may be sent to a participating pharmacy or to a pharmacy you choose where permitted.
Pharmacies are independent Third Parties and are responsible for dispensing, counseling, fulfillment, shipping, and pharmacy services.
Prescription products are generally non-returnable. Once a prescription has been issued or transmitted, or an order has been processed, you may remain responsible for fees already incurred.
Laboratory testing and diagnostics
Some services may require laboratory tests or diagnostic services. Laboratories and diagnostic providers are independent Third Parties.
Lab tests may produce false positives, false negatives, or inconclusive results. Delays, specimen issues, and handling or shipping problems can impact results.
Clinicians interpret lab results in their professional judgment and may recommend repeat or alternative testing or in-person care.
Products, shipping, and special handling
If products are shipped, delivery estimates are not guarantees. Shipping carriers are independent Third Parties.
You are responsible for providing accurate shipping information and for following storage and use instructions, including any temperature-sensitive handling requirements.
To the extent permitted by law, risk of loss may pass to you upon delivery.
Fees, billing, and payment authorization
You agree to pay all fees displayed at checkout or otherwise communicated to you in writing for your selected services (Fees).
Depending on the service, Fees may include clinical evaluation fees, membership or subscription fees, product costs, shipping and handling, lab fees, and applicable taxes.
You authorize us and our payment processors to charge your selected payment method for Fees, including recurring charges if you enroll in a subscription or recurring plan.
You are responsible for maintaining a valid payment method and for keeping billing details current.
Subscriptions and automatic renewals
If you enroll in a subscription or recurring plan, you authorize us to charge your payment method at the cadence disclosed at checkout until you cancel.
Unless otherwise disclosed at checkout, cancellation stops future renewals but does not reverse charges already incurred for the current billing period.
Plan terms, pricing, included services, renewal timing, and any minimum commitments may vary by program and will be disclosed at checkout or in written plan materials. Those plan-specific disclosures are incorporated into these Terms.
Cancellations
Cancellation methods and timing are described at checkout or in your account portal, if available.
If you cancel after services have begun, after intake has been reviewed, after a Clinician evaluation has occurred, or after a prescription has been issued or transmitted, you may remain financially responsible for Fees already incurred.
Refunds, returns, and payment disputes
All sales are final unless we determine, in our sole discretion, that a refund is appropriate, except where prohibited by law.
We do not offer refunds simply because you changed your mind, did not use the service, did not complete required steps, or did not achieve an expected outcome.
Because clinical services, administrative processing, and pharmacy fulfillment involve immediate costs and regulatory constraints, the following are generally non-refundable:
- clinical services already rendered
- administrative and processing fees
- prescription products once a prescription has been transmitted to a pharmacy or an order has been processed
- shipped products, opened products, or products that cannot be returned for safety or regulatory reasons
If you believe you were charged in error, charged twice, did not receive a shipment, or experienced another issue, you must contact support and provide documentation reasonably requested by us so we can investigate.
We may request proof such as receipts, screenshots, shipment tracking, pharmacy records, or other relevant documentation. We have the final decision on whether to approve a refund, except where prohibited by law.
If you initiate a chargeback or payment dispute without first contacting support and providing an opportunity to resolve the issue, we may suspend or terminate your account and provide information to the payment processor regarding the transactions and services delivered.
Privacy, health information, and security
Our collection, use, and disclosure of personal information is described in our Privacy Policy located on our website at https://patriotiv.com/.
Clinicians, medical groups, pharmacies, and labs may provide their own privacy notices or notices of privacy practices, which govern how they use and disclose protected health information.
We use reasonable safeguards designed to protect information, but no system is completely secure. You accept the risk of unauthorized access that may occur despite reasonable safeguards and agree to protect access to your devices, email accounts, and communication channels.
User Content, feedback, and permissions
If you submit User Content, you represent that you have the right to submit it and that it is accurate to the best of your knowledge.
You grant us and relevant Third Parties a worldwide, non-exclusive, royalty-free license to use, host, store, reproduce, modify, communicate, display, distribute, and otherwise process User Content as necessary to operate the Website or Platform, provide Services, facilitate telehealth communications, process payments, fulfill orders, provide customer support, improve services, and comply with legal obligations.
If you provide suggestions or feedback, you grant us the right to use it without restriction or compensation.
Acceptable use
You agree not to use the Website or Platform in a way that is unlawful, harmful, deceptive, or interferes with others.
Prohibited conduct includes:
- submitting false or misleading information
- impersonating others
- attempting unauthorized access
- disrupting the Website or Platform’s operation or security
- introducing malware or harmful code
- scraping, harvesting, or using automated means to collect data without permission
- using the Website or Platform to harass, threaten, or abuse others
We may investigate and take action, including suspension or termination, if we believe you violated these Terms.
Intellectual property and trademarks
The Website, Platform, and Content are owned by us or our licensors and are protected by applicable intellectual property laws.
We grant you a limited, non-exclusive, non-transferable, revocable license to access and use the Website and Platform for your personal, lawful purposes consistent with these Terms.
You may not copy, modify, distribute, sell, lease, sublicense, reverse engineer, decompile, or attempt to derive source code, except where prohibited by law.
All trademarks, logos, and service marks displayed on the Website or Platform are owned by us or our licensors. You may not use them without prior written permission.
Third-party services and links
The Website or Platform may link to or integrate Third Party services, including Clinicians, pharmacies, labs, shipping carriers, payment processors, and technology vendors.
We are not responsible for Third Party content, policies, practices, availability, delays, errors, or failures. Your interactions with Third Parties are governed by their terms and policies.
Disclaimers
The Website, Platform, Content, and administrative services are provided as is and as available.
To the fullest extent permitted by law, we disclaim all warranties, express or implied, including merchantability, fitness for a particular purpose, title, and non-infringement.
We do not guarantee that the Website or Platform will be available at all times, uninterrupted, secure, or error-free. We do not guarantee any specific medical outcome, response time, appointment availability, prescription decision, or that any service will meet your needs.
Limitation of liability
To the fullest extent permitted by law, we are not liable for indirect, incidental, special, consequential, or punitive damages, or for loss of profits, revenue, data, goodwill, or business interruption.
To the fullest extent permitted by law, our total liability for any claim arising out of or relating to the Website, Platform, or these Terms is limited to the amount you paid, if any, to us for the specific Services giving rise to the claim.
Some jurisdictions do not allow certain limitations, so some of the above may not apply to you.
Assumption of risk
You understand and accept the risks of telehealth and electronic communications, including technology failures, communication delays, and the limits of remote evaluation.
You agree to follow Clinician instructions and to seek in-person or emergency care when appropriate.
Indemnification
You agree to indemnify and hold us and our affiliates, officers, directors, employees, contractors, agents, and service providers harmless from claims, damages, liabilities, losses, and expenses (including reasonable attorneys’ fees) arising from your use of the Website or Platform, your violation of these Terms, your violation of applicable law, or your violation of the rights of a third party.
Termination
We may suspend or terminate your access to the Website or Platform if we believe you violated these Terms, we suspect fraud or misuse, required by law, or needed to protect safety or security.
You may stop using the Website or Platform at any time.
Termination does not eliminate your obligation to pay amounts already owed.
Governing law
These Terms are governed by the laws of the State of Arizona, without regard to conflict-of-law rules.
Dispute resolution and arbitration
Before starting a formal dispute, you agree to contact support and attempt to resolve the issue informally.
Except where prohibited by law, any dispute arising out of or relating to these Terms or the Website or Platform will be resolved by binding individual arbitration and not in a class, consolidated, or representative action.
Either party may bring an individual claim in small claims court if it qualifies.
Arbitration will be administered by a nationally recognized arbitration provider selected by the party initiating arbitration, under that provider’s applicable consumer arbitration rules. The arbitration will take place in Arizona, unless the parties agree otherwise or the arbitrator determines a different location is appropriate.
You and we waive the right to a jury trial.
You may opt out of arbitration by sending written notice within 30 days of first accepting these Terms. Your notice must include your name, the email address used for your account, and a statement that you are opting out of arbitration. Send the notice by email to info@patriotiv.com.
If any part of this arbitration section is found unenforceable, the remaining parts will remain in effect.
Time limit to bring claims
To the extent permitted by law, any claim must be brought within 1 year after the events giving rise to the claim. Otherwise, the claim is permanently barred.
Force majeure
We are not responsible for delays or failures caused by events beyond our reasonable control, including natural disasters, power or internet outages, system failures, labor disputes, supplier failures, public health emergencies, and government actions.
Assignment
You may not assign your rights or obligations under these Terms without our prior written consent.
We may assign these Terms in connection with a merger, acquisition, reorganization, or sale of assets.
Severability
If any provision of these Terms is held to be invalid or unenforceable, the remaining provisions will remain in full force and effect.
No waiver
Failure to enforce any provision of these Terms is not a waiver of that provision.
Survival
Sections that by their nature should survive termination will survive, including disclaimers, limitation of liability, indemnification, dispute resolution and arbitration, governing law, and other provisions necessary to give effect to these Terms.

We may provide notices to you by posting to the Website or Platform, by in-product notification, or by email to the address associated with your account.

Entire agreement
These Terms, along with any policies referenced in these Terms and any plan-specific disclosures provided at checkout, constitute the entire agreement between you and us regarding the Website and Platform.
