Privacy Policy
Last updated: Dec 2025
Overview
This Privacy Policy explains how Patriot IV LLC (Patriot IV & Pro health, we, us, or our) collects, uses, discloses, and protects information when you visit https://patriotiv.com/ or use any online services, tools, forms, or communications made available by or through our website (collectively, the Website and Services).
This Privacy Policy also explains how we coordinate with independent licensed clinicians and a clinical support organization that helps facilitate access to clinicians and supports prescription routing and care operations.
By using the Website or Services, you acknowledge this Privacy Policy. If you do not agree, do not use the Website or Services.
HIPAA and health information
Some information you provide through the Services may relate to your health. Depending on the circumstances, certain health information may qualify as protected health information (PHI) under the Health Insurance Portability and Accountability Act (HIPAA) or similar state health privacy laws.
Healthcare services are provided by independent licensed clinicians and or clinician groups (Clinicians). Clinicians may be subject to HIPAA and may provide a separate Notice of Privacy Practices that describes how they use and disclose PHI for treatment, payment, and healthcare operations.
Patriot IV & Pro health provides general and non-clinical support, such as account support, intake collection, scheduling support, and administrative communications. We may also work with a clinical support organization that supports care operations and prescription routing (Clinical Support Organization). Patriot IV LLC performs this Clinical Support Organization role.
Not all information collected through the Website is PHI. For example, information collected for general website analytics, marketing, or business communications may be governed by consumer privacy laws rather than HIPAA. In addition, some information may be outside HIPAA depending on how services are structured and which entity holds the information.
If there is a conflict between this Privacy Policy and a Clinician’s Notice of Privacy Practices for PHI handled under HIPAA, the Clinician’s Notice of Privacy Practices controls for that PHI.
